The Centers for Medicare & Medicaid Services (CMS) have released the 2026 premiums and deductibles for Medicare Parts A & B. For most people on Medicare, costs will go up – and it’s important to plan accordingly.

(2026) Medicare Part A — Hospital/Inpatient Coverage

  • The standard inpatient hospital deductible is $1,736 in 2026 (up from $1,676 in 2025).

  • Coinsurance for days 61-90 of hospitalization rises to $434 per day in 2026 (from $419).

  • Coinsurance for lifetime reserve days increases to $868 per day in 2026 (from $838).

  • For skilled nursing facility care (days 21-100 in a benefit period), the daily coinsurance will be $217 in 2026 (up from $209.50).

Note for Medicare Supplement (Medigap) clients:
If you are enrolled in a Medigap (also called Medicare Supplement) policy, you generally do not need to worry about the Part A deductible when admitted to the hospital because most Medigap plans fully cover that deductible. If you have one of those plans, your out-of-pocket for the Part A deductible should remain zero (assuming your Medigap is covering as expected).

(2026) Medicare Part B — Medical/Outpatient Coverage

  • The standard monthly premium for Part B will be $202.90 in 2026 (an increase from $185.00 in 2025).

  • The annual deductible for all Part B beneficiaries will be $283 in 2026 (up from $257).

Income-Related Monthly Adjustment Amount (IRMAA) for Part B for 2026

For higher-income beneficiaries, the Part B premium increases based on your Modified Adjusted Gross Income (MAGI). Below are the 2026 thresholds and premium amounts: Centers for Medicare & Medicaid Services

MAGI* (Individual) MAGI* (Joint) Monthly Premium
≤ $109,000 ≤ $218,000 $202.90
> $109,000 to ≤ $137,000 > $218,000 to ≤ $274,000 $284.10
> $137,000 to ≤ $171,000 > $274,000 to ≤ $342,000 $405.80
> $171,000 to ≤ $205,000 > $342,000 to ≤ $410,000 $527.50
> $205,000 to < $500,000 > $410,000 to < $750,000 $649.20
≥ $500,000 ≥ $750,000 $689.90

*MAGI = Modified Adjusted Gross Income from two years ago (as determined by the IRS). If you’re unsure whether IRMAA applies to you, it’s worth checking your Social Security or Medicare account.

✅ What This Means for You

  • Budget accordingly: If you pay the standard Part B premium, plan for approximately $203/month in 2026, and a $283 annual deductible.

  • High-income clients: If you are subject to the IRMAA surcharges, your monthly premium could be significantly higher — depending on your income level.

  • Medigap clients with Part A hospital coverage: Your Medigap policy likely takes care of the Part A deductible already, so your hospital entry cost remains covered.

  • Review your plan(s) annually: These changes underline the importance of reviewing your Medicare coverage each year (during Annual Enrollment Period or upon life changes) to ensure your plan still meets your needs and budget.
